LIABILITY WAIVER- I hereby waive, release and forever discharge Gail G. Faust/CCAC/MASTERPIECE MINISTRIES from any and all causes of action, claims, suits, damages and demands whether known or unknown, which I may have had, now have or may later have against and its respective officers, directors, employees, volunteers and representatives, arising out of or in any way connected to my participation in the dance ministry workshops/conferences, individual any of its respective classes, including but not limited to the Columbia Community Arts Centre and any affiliates associated with the OMS student dance workshops and classes A agree that I am physically fit to participate in the dance activities during the workshop. PHOTO AND VIDEO RELEASE-I agree that all pictures and video are released to Pastor Gail G. Faust, CCAC and Masterpiece Ministries for use in media and advertising. I understanding that the registration fee is non refundable. I understand and agree that if my behavior is detrimental or disturbing to others, I will be asked to leave the workshop without registration refund. All methods will be used to resolve participant issues.
